Unsafe Conditions - continued

The following unsafe conditions contributed to this incident:

  1. Rigid metal fuel lines within the fuselage with no provision for deformation during a survivable accident.
  2. Inadequate design, procedures, and training to maximize survivability after a survivable accident.
  3. Inadequate training for piston airplane pilots to transition into jet airplanes, which are less forgiving during approach and landing.
  4. The seating of the crew's three flight attendants away from emergency exits, which would preclude them from reaching those exits in time to assist in passenger evacuation.
