Lessons Learned - continued

The following lessons should be learned from this event:

  1. Post crash survivability is greatly compromised by uncontrolled fuel leaks.
  2. Proper application of changed requirements (e.g., flexible fuel lines) is dependent upon understanding the origin of the requirements.
  3. Special consideration must be given to fuel system component location in the design process, because of the potential of a compromised fuel system to adversely affect the survivability of minor accidents/incidents. Airplane survivability needed additional attention.
  4. Flight crew training for transitioning to jet airplanes was inadequate.
