Lesson 1 - Systems Approach to Safety Summary
This concludes Lesson 1 of the Applying Lessons Learned from Accidents course. Some key points to remember:
- Don't expect the analysis and findings for all accidents to be the same. Information you can obtain/determine will depend on the accident, when it occurred, where it occurred, what's available, etc. There is no standard.
- FAR 25.1309 did not exist when some of these accidents occurred.
- If something doesn't "look right" to you, it probably isn't.
- Even if the original design is great, subsequent design modifications can have unintended consequences (e.g., adding an in-flight entertainment system).
- Sometimes immense political pressures are brought to bear on the FAA. Sometimes you will be pressured to do what is not right. Don't forget the importance of doing what is right, and holding the line in spite of the political pressure to do otherwise.
- Corrective actions can introduce additional problems and must be thoroughly evaluated by the correct disciplines and assessed for possible unintended airplane level effects.
